To lay the correct groundwork, we asked podiatrist Dr. Steven Gershman to explain about the condition. 

“Diabetic Charcot foot is a progressive neuropathic arthropathy - a degeneration of the joints and bones in the foot. Joint dislocation and collapse mainly in the midfoot can occur. It can be very disfiguring along with creating an unstable foot prone to open wounds or ulcerations on the bottom of the foot,” says Dr. Geshman. 

“Charcot foot is caused by nerve damage from diabetes, leading to loss of sensation, which causes overuse of joints and eventual breakdown or dislocation, or both. In addition, there is an increase in blood flow to the bones from the nerve damage, which washes out and weakens the foot bones,” he explains.

Dr Gershman adds that “Initially, the patient presents with a swollen warm foot that is not infected. Untreated, it will progress to the breakdown of the joints and dislocation and permanent deformity of the foot. 

Initial treatment is complete immobilization of the foot, ankle or lower leg in a cast or solid boot. In addition, the patient should be non-weight bearing until the foot cools down and stabilizes. This can take months in severe cases,” he says.

What type of footwear is best for Charcot foot?

“After the cast or boot is removed, the person can return to shoes, sometimes with a brace if deformity has damaged the foot structure. Shoes should be appropriate to the level of damage (if any) that Charcot has caused,” clarifies Dr. Geshman. 

He goes on to specify that “If the foot is structurally sound and intact, then a well made supportive shoe is worn. A quality insert to support the foot is important also. The insert may need to be custom molded if there is a risk of ulceration due to bony protrusions.” 

“The shoe should be designed with extra depth and a round, wide toe box to avoid pressure on the toes,” says Dr. Geshman. “As there is a lack of sensation, the shoe should avoid rubbing on any part of the foot so the inside should be soft and seamless. The leather should be soft but also strong. A rocker outsole can be helpful. Opt for shoes that are made to match these qualities as they are highly recommended for diabetic neuropathy.”

Dr. Gershman also notes, “If a brace such as an AFO (ankle-foot-orthosis) is required, the shoe should be extra wide and deep, and in some cases needs to be stretchable.”

“If the foot has developed significant deformity from Charcot, then a true custom molded shoe will be required,” he concludes.

What is the gold standard for Charcot foot?

“As noted earlier the gold standard is immobilization and non-weight bearing initially”, says Dr. Gershman. “This can be difficult for patients as it impacts their lives significantly. A scooter that allows the affected leg to completely rest can be helpful.”

He goes on to explain, “After the initial period, shoes and inserts that accommodate any deformity that has occurred are critical. Avoiding ulcers is the goal as once a wound or ulcer opens up, it can take months to heal and sometimes leads to amputation.”

Does Charcot foot get worse?

According to Dr. Gershman, it can.

“If proper initial treatment is not followed and permanent deformity occurs, the foot is at high risk of ulcers. Ulcers are very difficult to heal in these cases as the underlying deformity, such as protruding bone on the bottom of the foot, is a constant factor. Also, diabetics don't heal well due to the disease itself. Infection is a constant risk as diabetics are very prone to infection,” he adds.

“Charcot foot is a very serious condition, but can be managed if treated early and aggressively. The patient has to stick with the often difficult treatment plan. However, the end result of proper treatment is a fully functioning foot. If not, the condition will worsen and can lead to amputation,” summarizes Dr. Gershman.
